Our Commitment
Pathways to Balance Counseling & Supervision, PLLC is required by the Health Insurance Portability and Accountability Act (HIPAA) to protect the privacy of your protected health information (PHI), provide you with this notice of our legal duties and privacy practices, and follow the terms of the notice currently in effect.
Uses & Disclosures of PHI
We may use and disclose your PHI for the following purposes without your written authorization:
- Treatment — to provide, coordinate, or manage your care.
- Payment — to bill and collect payment for services rendered.
- Health Care Operations — for quality assessment, training, supervision, and other administrative functions.
- Required by Law — when disclosure is required by federal, state, or local law.
- Public Health & Safety — to prevent or lessen a serious threat to your health or the safety of others, or to report suspected abuse of a minor or vulnerable adult.
- Judicial Proceedings — in response to a court order or other lawful process.
Other uses and disclosures will be made only with your written authorization, which you may revoke at any time in writing.

- Request an amendment to your PHI.
- Request an accounting of disclosures.
- Request restrictions on certain uses and disclosures.
- Request confidential communications by alternative means.
- Receive a paper copy of this notice.
- File a complaint without fear of retaliation.
